New Requirement for Reporting Your Lost or Stolen Visa

Letter from Migration Now Required

Effective September 17, 2007, individuals whose valid United States visa is lost or stolen will have to provide a letter from the Director General of Migration for the Dominican Republic when applying for a new visa.

Report Your Visa Lost or Stolen

If your valid U.S. visa has been lost or stolen, immediately notify the U.S. Embassy Santo Domingo Consular Section.  Please file your report in person at window 7, Monday through Thursday, at 11:00 a.m.  You must have the following documents:

  1. Letter from the Director General of Migration for the Dominican Republic showing all your Dominican Republic exit and entrance dates for the last two (2) years;

  2. Report from the National Police indicating that your passport was lost or stolen;

  3. Letter of Explanation including the date, circumstances, and location of the visa’s loss or theft.  Make sure you include all your biographic information including your complete name, cedula number, date of birth, and place of birth; and

  4. A Photograph taken recently, original, front view, 2 x 2, in color.

Applying for a New Visa

If your visa was lost or stolen and you want to apply for a new visa, please schedule an appointment through our service at (809) 200-3232.  You will not qualify for a renewal appointment since you no longer have your visa in your possession.  You must bring all the required documents (listed above) with you to the interview.  There is no guarantee that that you will be entitled to another visa.  Please remember that it is your responsibility to safeguard your visa against loss or theft.
