Take Care When Completing Nonimmigrant Visa (NIV) Application Forms

All NIV applicants are now required to submit applications that have been filled out on a computer.  The U.S. Embassy no longer accepts applications that have been filled out in writing.  YOU DO NOT NEED TO PAY FOR ASSISTANCE IN FILLING OUT YOUR FORMS.  You can use any computer connected to the Internet and a printer using these easy steps:

  1. Find the form on our website, http://www.usemb.gov.do/Consular/consular-e.htm in the nonimmigrant visa section.  The entire website and forms are available in Spanish – just press the Spanish language tab on the top bar.  You are responsible for responding correctly to each question, so please look for the Spanish language version of forms if you do not understand English.

  1. Complete the form online.  Don’t leave any sections blank.  Then print it.  The embassy does NOT receive the information electronically – your information is saved in the bar code on the copy that you present when you come for your interview.  Our computers simply read the bar code.

  1. Read the form you just printed carefully and make sure it is correct before you sign it.  If the consul discovers during the interview that your written responses are not correct, it may create doubts about your credibility.  Correcting mistakes is easy – just start over.  When you print the new form, the new bar code will have the corrected information.

  1. The information in your passport must also be correct and must match your birth certificate.  If there is an error in the spelling of your name or in your date of birth, you need to obtain a new passport with correct information – we cannot put a visa in a passport with erroneous information.
