Complete your US Passport Application online

The application for US passports can be completed and printed through an on-line form at http://www.travel.state.gov/passport/forms/forms_847.html.  A ‘wizard’ will prompt you for information and walk you through the process of entering data that will subsequently be printed on your passport application.  After answering all the questions, you hit ‘submit’.  At the top of the next page appearing on your screen, you click on ‘create form’.  This will generate the bar-coded passport application form for you to print out.  This printed application form will have a barcode containing your information (similar to the barcode on the application for a non-immigrant visa) which we can scan when you bring it to the Consular Section.

The barcode will enable the passport office to process passport applications quicker and more efficiently and it will help us get you in-and-out of our waiting room in less time.   We plan to require all passport applicants to utilize the on-line passport forms that print with a barcode, probably beginning around the first of July.  We will provide ample advance notice prior to implementing this requirement.   However, as an incentive to use the new bar-coded forms now, individuals with appointments who utilize the bar-coded forms will be seen and processed immediately.

Remember to bring your completed form with you to the Consular Section’s passport office on the date of your appointment.  Remember that adults who are renewing passports using form DS-82 do not need an appointment.   For complete information regarding passport procedures at the Consular Section in Santo Domingo, please review the web page http://www.usemb.gov.do/Consular/ACS/passports-e.htm.
