Embassy of the United States of America Consular Section Santo Domingo, Dominican Republic INFORMATION CONCERNING THE MEDICAL EXAMINATION AND VACCINES The Medical Examination The Immigration and Nationality Act requires that all immigrant visa applicants and certain nonimmigrant visa applicants, regardless of age, undergo a medical examination before receiving a visa. The doctors selected by the Consular Section are the only doctors authorized to conduct medical examinations for visa applicants. The doctors’ office is called Consultorios de Visa and is located in Santo Domingo, on Avenida Independencia 202, Condominio Santa Ana, First Floor (next to Clínica Abréu). You may go to Consultorios de Visa any working day between 7:00 a.m. and 10:00 a.m. for your medical examination. If you are an immigrant visa applicant, you must take the enclosed immigrant visa appointment letter to Consultorios de Visa. If you are applying for a K visa, please take the enclosed instruction sheet to Consultorios de Visa. Because the results of the medical examination can take several days, we recommend that you go to Consultorios de Visa at least five (5) working days before you go to the Consular Section for your visa interview. Please present to the Consultorios de Visa receptionist this information sheet, your appointment letter, your passport, three (3) color 2” x 2” photographs and your national identification card. These same requirements apply to all members of your family who plan to accompany you to the United States. If you have ever had any disease or serious injury, it will be useful for you to take any relevant X-rays or medical reports to the clinic with you. You must pay at Consultorios de Visa US$100.00 or the equivalent in Dominican pesos for each adult examined (US$30.00 for each child under age 15). This amount includes X-rays, blood tests and the medical examination itself. If the examining physician considers that other tests are necessary, you must pay for these separately. Children under 15 years of age do not need to have X-rays taken nor blood tests conducted, unless the doctor considers them necessary. Do not fast before your medical examination. You should eat normally. In addition, if you are taking any medication, you should continue to take it normally. Consultorios de Visa generally will give the medical examination report directly to you for you to take to the Consular Section on the day of your visa interview. In some cases, however, Consultorios de Visa will send the examination report directly to the Consular Section. Consultorios de Visa maintains a website with information concerning the medical examination process. The address of the page is http://www.consultoriosdevisa.com/english.html. Vaccines The Immigration and Nationality Act requires that all immigrant visa applicants obtain certain vaccinations before receiving an immigrant visa. If you are applying for a K visa, vaccines are optional. However, it is often more convenient if you are vaccinated while still in the Dominican Republic. The doctors selected by the Consular Section are the only doctors authorized to conduct medical examinations for visa applicants. The doctors’ office is called Consultorios de Visa and is located in Santo Domingo on Avenida Independencia 202, Condominio Santa Ana, First Floor (next to Clínica Abréu). The doctors must verify that each immigrant visa applicant has met all vaccination requirements. The cost of the vaccinations varies according to the age of the applicant. The applicant is responsible for paying the fee upon arrival at Consultorios de Visa. Effective in June 1st 2008, these are the new vaccines and their new prices.
*Between February 22nd and August 20th the influenza (flu) vaccine is not applied If you pay in Dominican pesos, the exchange rate will be the market rate offered on the day you pay. Only the doctor can determine which vaccinations are medically
appropriate for you, based on your age, medical history and current
medical condition. If the doctor determines that you do not need any
vaccinations or that you need only some of the normal vaccinations, Consultorios de Visa will refund all or part of the fee listed above.
